Does your service replace my front-office staff?

​No. We know that your front office staff is essential for your medical office to run smoothly and efficiently. This can be difficult when they are bombarded by an overwhelming amount of calls. Our nurses handle between-visit chronic care management (CCM) calls, these can include medication questions, care-plan check-ins, lifestyle coaching, so that your front desk staff can focus on in-office patients. We become an extension of your team, not a replacement.

What will your services cost me?

There are no out-of-pocket costs associated with our services. CCM is a Medicare-approved program, increasing your overall office revenue. Please schedule an appointment for more information on billing and reimbursement.

Are your services customizable to my practice’s needs?

Absolutely. Our nurses are able to adapt their outreach calls; so if there are any specific reminders that you would like conveyed to your patients, we can meet your needs! Examples would include, reminders to schedule their annual appointments (diabetic, eye exam, foot exams ), to get their flu vaccine during flu season, to bring an updated medication list to their next appointment.

How fast can we launch?

Most practices are live within 30 days: contract signed → EMR access → patient consent and enrollment→ first monthly outreach. We supply a turnkey onboarding checklist so nothing falls through the cracks.
